Prospective and comparative study of functional outcome of intertrochanteric fracture of femur treated with dynamic hip screw (VS) proximal femoral nail

Abstract: Without proper management, it is a sensitive issue to regaining quality of life is difficult for fractures of the lower limb. Re-establishing the pre-operative status & mobility without any medical complications is the main goal of management for any fracture. This study was done as a prospective comparative study among 44 subjects who underwent DHS or PFN for inter-trochanteric fracture in SSSMC&RI, Ammapettai. The groups were compared on basis of blood loss, surgery time, post-operative ambulation, fluoroscopic exposure, % union at 14 weeks and Harris-hip score at 6 months. Most of the subjects in both groups had Boyd/Griffin classification-type 2 injury. In each group, there is the history of Road Traffic Accident (11), slip and fall (9) and heavy object fall (2) leading to fracture. Amid DHS group shortening of limb (2), bed sore (2), delayed union (1), lag Screw pulls out (1) and varus angulation (1) as complications. Amid PFN group malunion (2), delayed union (1), limb shortening (1) and infection (1) as complications. PFN group had lesser reduced blood loss, surgery time, less fluoroscopic exposure, early post-operative ambulation. PFN has a better % union at 14 weeks and Harris-hip score at 6 months. The differences were statistically significant.
