Prospective evaluation of fixed-angle locking compression plates in the management of volar Barton's fracture

Abstract:

Objective: Volar Barton's fracture is an inherently unstable injury of the distal radius, often associated with volar displacement or subluxation of the wrist joint. The primary objectives in managing this fracture are to achieve precise anatomical reduction, ensure stable fixation, and facilitate early restoration of hand function while minimizing long-term complications. Successful bone healing requires a minimal fracture gap, stable fixation, and preservation of vascular integrity. Locking plates are commonly favored in such cases, as their design allows for stable fixation with reduced compressive force on the periosteum, thereby helping to maintain blood supply and promote optimal healing conditions.

Patients and Methods

Source of Data: A prospective study was conducted in the Department of Orthopaedics at Bhaskar Medical College and General Hospital, Moinabad, between January 2021 and June 2022. The study included 20 patients diagnosed with volar Barton's fracture. All patients underwent open reduction and internal fixation using a 3.5 mm fixed-angle locking compression plate.

Result: The study population comprised 12 male and 8 female patients, ranging in age from 20 to 60 years, with a mean age of 34.65 years. Patients were followed up for a period ranging from 6 to 24 months. Functional outcomes were assessed using the Gartland and Werley demerit scoring system, with results showing 50% excellent, 40% good, and 10% fair outcomes.

Conclusion: Fixation of distal radius fractures-particularly intra-articular volar Barton's fractures using a locking compression plate yields satisfactory outcomes for both patients and surgeons. Locking plates offer stable fixation, which contributes to favorable functional results and may facilitate an earlier and more effective return of wrist mobility.
