Comparative study between clinical and functional outcomes of DHS and pfna2 in management of Boyd and griffin type 1 and 2 intertrochanteric fractures

Abstract:

Introduction: Intertrochanteric fractures are common in the elderly population and are associated with significant morbidity and mortality. This study aimed to compare the clinical and functional outcomes of dynamic hip screw (DHS) and proximal femoral nail antirotation Asia (PFNA2) in the management of Boyd and Griffin Type 1 and Type 2 intertrochanteric fractures. 
Methods: A prospective observational study was conducted on 46 patients (23 in each group) with intertrochanteric fractures treated with either DHS or PFNA2. Intraoperative variables, postoperative complications, pain assessment, functional outcomes, hospital stay, and recovery data were compared between the two groups. A subgroup analysis based on fracture type was also performed. 
Results: The PFNA2 group had significantly shorter intraoperative time (78.0 +/- 7.1 vs. 102.7 +/- 7.5 minutes, p<0.001), less blood loss (116.7 +/- 38.5 vs. 258.7 +/- 51.1ml, p<0.001), lower incidence of postoperative complications (26.1% vs. 65.2%, p=0.008), lower pain scores on postoperative day 3 (5.0 +/- 1.6 vs. 5.7 +/- 2.1, p=0.007), and higher total Harris Hip Score at 3 months (83.0 +/- 10.1 vs. 57.0 +/- 24.6, p<0.001) compared to the DHS group. The subgroup analysis showed consistent benefits of PFNA2 in both Type 1 and Type 2 fractures. 
Conclusion: PFNA2 demonstrated superior clinical and functional outcomes compared to DHS in the management of intertrochanteric fractures, with shorter intraoperative times, less blood loss, lower incidence of postoperative complications, better early postoperative pain control, and improved functional outcomes. These benefits were consistent across both stable and unstable fracture types.
