Evaluating functional recovery after uncemented bipolar hemiarthroplasty in geriatric femoral neck fractures

Aim and Objective: To evaluate the functional outcomes of uncemented Bipolar hemiarthroplasty in displaced femoral neck fractures in geriatric age group.
Background: In geriatric patients the incidence of femoral neck fracture is one of the most common traumatic injuries indicated for hospitalization. Approximately 95-97% of these fractures are seen in female patients above the age of 50 years. The use of osteosynthesis in geriatric patients leads to complications such as non-union and osteonecrosis. Modular bipolar hemiarthroplasty is helpful in eliminating such complications with decrease in multiple surgeries. This treatment modality provides good clinical results and achievement of remarkable mobility, functional ability and pain control.
Material and methods: A retrospective study was conducted after ethics committee approval at GAIMS, Bhuj from November 2023 to September 2024. 40 participants of both genders were included by simple random sampling method based on inclusion criteria. Patients with displaced femoral neck fracture treated with uncemented bipolar hemiarthroplasty were included in the study. Clinical outcome was assessed at the periods of 6 weeks, 3 months and 6 months using Harris Hip scoring system. Statistical analysis was done with p-value set at 0.05.
Result: Out of 40 subjects, 22 were female patients and 18 were male patients. The mean age of participants included in the study was determined as 66.88±2.49 years. In the comparison between 3 months and 6 months, 82.5% (n=33) of participants remained unaided, 5.0% (n=2) worsened from unaided to aided, and 5.0% (n=2) improved from aided to unaided. 7.5% (n=3) remained aided at both time points. The McNemar test value was 1.000, with a p-value of 1.000, showing no significant difference in mobility status between these two time points. It was determined that average HHS was 86.7±6.95, which suggested the range of 70 - 98
Conclusion: Uncemented Bipolar hemiarthroplasty gives promising treatment outcomes like sufficient functional outcome in daily activities, minimal hospital stays, pain control and fewer post-surgical complications.
